CommuniCare Advantage

CommuniCare Advantage will cover home infusion therapy if:

• Your prescription drug is on CommuniCare Advantage formulary or you have a formulary exception;
• CommuniCare Advantage has approved your prescription drug for home infusion therapy; and
• Your prescription is written by an authorized prescriber.

In cases where you need home infusion therapy, a nurse case manager will be working with you and your doctor to make sure that you have the care you need. If you have any questions regarding home infusion pharmacy services, please contact Member Services at 1-888-244-4430.

All network home infusion pharmacies provide services throughout San Diego County.

Community y Más

Community y Más will cover home infusion therapy if:

• Your prescription drug is on Community y Más formulary or you have a formulary exception;
• Community y Más has approved your prescription drug for home infusion therapy; and
• Your prescription is written by an authorized prescriber.
